  1. Choose the exclusion period. Decide whether the request should run for six months, one year or five years.
  2. Prepare a government-issued ID. The current online instructions require an image of an official ID.
  3. Complete the identity check. PAGCOR says the online application includes a selfie. A non-personal submission may require confirmation through a selfie or video call.
  4. Review every declaration. Confirm the scope, duration and treatment of personal information before submitting.
  5. Keep a private record. Save the submission date and confirmation securely, without posting the ID, form or reference publicly.

Family exclusion is a separate route for a spouse, parent or child aged at least 18 who seeks an exclusion order for a loved one. PAGCOR currently lists family-exclusion periods of six months, one year and three years.

The required documents depend on the relationship. They can include IDs for the applicant and respondent, a recent photograph, a birth certificate or marriage certificate, and a completed family-exclusion form. Incomplete requirements are not processed according to PAGCOR's published procedure.
